Benefits Administration

Eligibility – Track employee eligibility with notification reporting to both Company and employees within designated enrollment timelines.

Enrollment – Assists with employee carrier enrollments, form auditing, follow-up and onboarding, including payroll deduction set-up.  Collection and electronic archiving of benefit enrollment forms.

Premium Reconciliation – Matches enrolled participants and participant deductions with Carrier invoices, including tracking and follow-up with the Carrier/Broker on adjustments, adds/deletes, and qualifying events.

COBRA – Service includes certified written notice, premium collection and remitted payment to carriers.

Section 125 Premium Only Plan – MosaicHR provides the Section 125 Election Form in the enrollment packet, administers and maintains plan compliance, including calendar year election notifications to all participants.

ACA – Consulting, compliance tracking and forecasting

Benefit Processes – Benefit delivery automation and standardization streamlines and improves communication throughout the organization.

Medical Support Orders – Includes initial responses, enrollment confirmations, and communication with appropriate court jurisdictions.

Medicare Part D – Provide required annual reporting to Centers for Medicare and Medicaid Services.

FMLA or Extended Leave – Assists Company in overall administration with initial employee notice, coordination of benefit premium collection, arrears payments and ongoing maintenance during the employee leave period.
